    For the first time since I opened 9 years ago, I am wondering if I should keep my business open.  I've lost a ton of patients from the Obamacare reforms, because I'm not in a big network.  I have seriously been considering opening just one day a week in a small space that allows me to do my new passion:  genetics, plus see some family practice patients.  But I'd like to be able to take insurance, and know that their contracts usually require the office be open 20 or more hours a week.  I am wondering if you now of any niche practices and how they made this work?  Do insurances ever allow for exceptions to their contracts?

    I don't have experience with this question.  It does seem to me however that starting by asking your contract rep from one of your bigger insurers might not be too tricky.  If you see Molina patients, I can give you the contract rep information for them as I've been emailing with them.  For Regence and Premera I'd have to look it up.

    The most useful thing is to be part of a PHO- now mostly turned ACO     Going things completely on your  o wn  seems a  tall fence to scale.  Is there no group of other like minded NPs  ?
    Not the same of course as being employed  Perhaps you should take a  part time job anyway and then do the genetics thing you love on your own

    In my region, most  PPO's and medicare do not insist on "20 hours a week".  HMOs yes.
